Can you use Butcher’s Broom for Hemorrhoids?

Butcher’s broom root contains neoruscogenin and saponin glycoside ruscogenin, both have been found to cause veins to contract and have anti-inflammatory and astringent properties. This led to the use of Butcher’s Broom for hemorrhoids.

Patients that used butcher’s broom for hemorrhoids showed significant improvement in the following symptoms after 7 days:

  • pain
  • local signs
  • itching
  • burning
  • overall severity

Pregnant women often suffer from hemorrhoids and treatment with surgery is very difficult. Butcher’s broom caused an improvement in hemorrhoids symptoms with a high level of safety and no side effects on the baby.

The use of butcher’s broom for hemorrhoids has been approved by the following institutions:

  • The German Commission E: approved the use of butcher’s broom rhizome extract for hemorrhoids
  • European Scientific Cooperative on Phytotherapy (ESCOP): approved the use solid or liquid extracts of dried butcher’s broom rhizome for hemorrhoids.

What is the Butcher’s Broom Dosage for Hemorrhoids?

The Butcher’s Broom dosage for Hemorrhoids depends on the medication used:

  • Cyclo 3 Fort: 6 capsules daily for 3 days, followed by 4 capsules daily for 4 days.
  • Fabroven: 3 to 12 capsules daily for 3 days, divided in 3 doses. In some cases, followed by 4 capsules daily divided in 2 doses.

Fabroven and Cyclo 3 Fort both have a combination of ruscus extract, hesperidin methyl chalcone and ascorbic acid (vitamin C). In a study, 124 patients were given Cyclo 3 Fort. After these 7 days of treatment, the results were the following:

  • 69% of the patients rated butcher’s broom as having a good or excellent effect
  • 75% of the treating physicians rated it similarly

  • Health condition: acute hemorrhoidal disease (grade I, II and III).

Method used

The patients were treated with 3 to 12 capsules of Fabroven, divided in three doses every 24 hours the first three days of treatment, and in some cases, four capsules divided in two doses thereafter.

The studied symptoms were:

  • painful symptoms (discomfort, sensation of heaviness, burning, pruritus, tenesmus)
  • accompanying symptoms (rectal bleeding, altered intestinal motility, abdominal pain)
  • local signs (prolapse, congestive state, inflammation)
  • over-all severity score of symptoms

Results

Data analysis showed a statistical significant decrease in all scores between zero and seven days.

The therapeutic efficacy was proven in 90% of patients and only 5% reported some adverse reaction.

The pain evaluation score showed a decrease of five points on the average:

  • Functional symptoms decreased nine points
  • Local symptoms 3.5 points
  • Accompanying symptoms decreased 1.5 points

Conclusions

The treatment with Fabroven decreases significantly the pain, functional symptoms and inflammation in hemorrhoidal crisis of patients with disease grade I, II and III.

  • Tablets or capsules of a 4:1 concentrate: 200 mg 2–3 times daily
  • Tablets or capsules of a 10:1 to 20:1 concentrate: 37.5mg twice daily (total 75mg daily)
  • 1:2 herb-to-liquid ratio liquid extract: 3–6 ml daily
  • 1:5 herb-to-liquid ratio liquid extract: 7.5–15 ml daily
  • Topical cream extract: 64-96 mg daily

The following Butcher’s Broom dosages have been used for vascular conditions:

  • Chronic venous insufficiency (poor circulation): capsules of 150 mg butcher’s broom dried root extract (combined with 150 mg hesperidine and 100 mg ascorbic acid) 2–3 times daily, OR 7-11 mg of standardized ruscogenin.
  • Chronic phlebopathy (ulcers on the lower limbs): 16 mg extract daily.

However, there is not enough scientific evidence to provide an optimal Butcher’s Broom dosage.

  • Title: Efficacy and safety of a Butcher’s broom preparation (Ruscus aculeatus L. extract) compared to placebo in patients suffering from chronic venous insufficiency.

Method used

Extracts from Butcher’s broom rhizome (Ruscus aculeatus) have been widely used in the oral treatment of lower leg edema in patients with chronic venous insufficiency.

The study was carried out according to the guidelines for testing drugs for chronic venous insufficiency.

Results

There were significant differences between the treatment groups ruscus and placebo, for the change of:

  • leg volume after 8 and 12 weeks of treatment
  • ankle and leg circumferences after 8 and 12 weeks of treatment
  • changes in subjective symptoms, heavy tired legs and sensation of tension (week 12)

For the changes in the symptoms heavy lower legs, sensation of tension, and tingling sensation a significant positive correlation with the changes in leg volume was shown. 

Overall assessment of efficacy was significantly better for ruscus extract compared to placebo. Overall tolerability for both treatments was assessed as good and very good. 

Conclusions

Considering the study duration of three months it is concluded, that ruscus extract, in the recommended daily dosage according to the German monograph, is a safe and effective treatment for patients suffering from chronic venous insufficiency.

Her diabetic ketoacidosis was complicated by hyperkalemia and acidosis with a pH of 7.02. After management with intravenous fluid, insulin, and calcium gluconate, her condition stabilized and she was discharged several days later 

Conclusions

This case represents a small but potentially serious public health concern in a diabetic woman taking Butcher’s Broom.
